Print a Blank Numbered Quiz Template in One Click
Sometimes you just want a numbered answer sheet and nothing else. This generator prints a clean, numbered quiz template with the number of questions, answer-line style and header fields you choose.
Pick between 1 and 50 numbered questions, choose a single underline, three ruled lines or a blank box for each answer, and download a print-ready A4 or US Letter PDF. Write the questions in by hand, read them aloud, or pair with a separate question sheet.
Designed for teachers, tutors and homeschool families who want a blank answer sheet they can reach for on any topic without fiddling with formatting.
Why use this quiz template generator?
A blank answer sheet is endlessly versatile. Use it for:
- read-aloud pub-quiz-style starters
- weekly spelling tests
- times-table speed tests
- vocabulary recall checks in MFL
- end-of-lesson plenaries
- house quizzes and enrichment sessions
- cover-lesson tasks a non-specialist can read aloud
One template, countless uses — the teacher provides the questions, the template provides the structure.
What you can customise
The generator keeps the options focused:
- Title: defaults to "Quiz" — rename for specific events ("Friday Spelling", "House Quiz Round 2")
- Instructions line: wraps across the page above the first question
- Question count: 1–50 numbered rows
- Answer-line style: single underline, three ruled lines, or blank box
- Name and Date header: on or off
- Paper size: A4 or US Letter
Row height auto-adjusts so the template always fits neatly on a single page.
Notes and limitations
- This is a blank template — the tool does not generate questions. Use a separate question sheet, read aloud, or project on a whiteboard.
- High question counts shrink the row height. For long-answer questions, pick the blank box line type and fewer questions.
- There is no built-in answer key — keep one on a separate teacher copy.
- Print at 100% scale for consistent spacing.

Answer-line style options
Single underline
One underline per number. Best for short-answer questions like spellings, vocabulary translations and factual recall.
Three ruled lines
Three stacked lines per number. Best for short explanations or two-sentence answers.

Worked example
A primary teacher runs a weekly Friday spelling test. She generates a quiz template titled "Friday Spellings" with 20 questions, single underline answer lines, Name and Date on, on A4 paper.
The PDF prints 20 numbered underlines across a single page. On Friday she reads each spelling aloud; pupils write the word on the corresponding line. Because the template is always the same, pupils know the routine and settle quickly. She prints a fresh stack every Monday so it's ready for the week.
Methodology
The engine prints a numbered list with the answer-line style of your choice beside each number. Row height is calculated from the question count so the template always fills a single page. The title and instructions line wrap above the first question. The shared branded template adds the Name/Date header, footer watermark and QR code for consistency with every other printable on the site.
Helpful preset ideas
- 10 questions, single underline — weekly spelling or recall test
- 20 questions, single underline — general knowledge quiz
- 12 questions, three ruled lines — short-answer comprehension
- 8 questions, blank box — maths working or diagrams
- 30+ questions, single underline — times-table speed test
Best ways to use the template
- Keep a stack pre-printed so quizzes can start in seconds.
- Read questions aloud at a measured pace for Years 3–6.
- Project the question list on the board for silent working.
- Use the same template every week so pupils know the drill.
- Use the blank box version for maths to make working visible.
Designed for A4 and US Letter printing
Both A4 and US Letter are supported. The engine rescales row spacing to fit the chosen page size while keeping the branded header consistent.
