A voucher, not a certificate
An achievement certificate is landscape, framed and meant to be kept. A voucher is cut out, handed over and redeemed — so this is small, comes two or three to a page, and carries the three things a voucher is useless without.
Value, expiry, serial
The value is the big line, and it takes words as readily as money: one free haircut, a day out, an hour of babysitting. The expiry protects you from a voucher surfacing four years later. The serial is what lets you tell a redeemed voucher from a photocopy.
The serial is a format, not a number
The sheet prints No. PW-____-____ for the issuer to complete by hand. Generating a number here would give every download of the same settings the same "unique" reference, which is worse than having none — so the space is printed and the numbering is left to whoever issues them.
Cutting them out
A dashed guide runs between the vouchers. Card stock takes a pen better than paper and survives a wallet, which is where most of these end up.
